工作內容: 1.擬定照顧計畫與服務連結 -追蹤個案與各項資源服務之連結情形。 2.照顧管理 (1)依長照需要者需求或長照需要變化調整照顧計畫。 (2)執行服務計畫。 (3)追蹤長照需要者與各項服務之連結情形。 (4)每月進行服務品質追蹤。 (5)接受長照需要者及其家屬有關長照服務諮詢、申訴及處理。 (6)協助長照需要者或其家屬其他資源連結 。 3.計畫撰寫與執行、訪視紀錄撰寫與核銷。 4.協助單位行政工作 5.單位臨時性活動機動配合 6. 社區長照宣導及講座辦理 7.接受長照使用者及其家屬服務諮詢,申訴及問題處理。 8.經營與管理服務單位(B單位)之合作關係(督導品質、輔導、合作配合) 9. 主管交辦事項 設有激勵獎金,鼓勵同仁配合A單位業務
